Subject: My "aching back"

To all,
    I was just recently ex-rayed by an out of town orthopedic surgeon to see
the status of my aching back.  To my relief, my scoliosis is fairly mild. 22
thoracic, 20 lumbar.  There is a herniated disc at the L-5/S-1 site.  He
said there is a 75% chance that I would get relief from the sciatica by
having it removed.  Is this better done by an orthopedic or neurosurgeon?  I
am thinking I will have this done right after Christmas.
    Now, the other problem.  I have been experiencing severe pain in the
upper back between the shoulders for the past two years.  My entire rib cage
hurts constantly!  He says I have 65 degree kyphosis.  At my age, (47) he
felt surgery would be much too drastic.  He used the term "keel hauled."
This is when sailors were punished by being thrown off the bow of the boat
and drug alongside.  I don't think he was trying to be funny.  He said a
colleague's wife had the surgery and at four months post-op she was wishing
she had not. He also saw some arthritis and osteosporosis.   So, if surgery
is not advisable for me, what do I do about the excruciating pain? I do have
an appointment with my pain management doctor Sept 27.  I'm not sure what to
do or what to ask my pain management doctor for.  I've been through PT
twice, had epidural injections and tried Neurontin.
    I am also the woman who posted about insomnia a few weeks ago.  I really
appreciate all you who posted your suggestions.  I have an appointment with
the sleep specialist Sept 28.  So, hopefully, we can resolve the sleep
problem.

  I would think that you should demand that breast shields are used when your 
 daughter has x-rays taken.  If the x-rays are taken P/A (your daughter is 
 facing the machine, and has her back against the film), you should discuss 
 this matter with the doctor and see about having the order changed to A/P 
(your daughter is facing the film, and has her back towards the machine).  
You 
 will find that most of the newer machines have the shields for the 
reproductive organs built into the machine (I believe a complete lead apron 
not held correctly would defeat the purpose of scoliosis x-rays, and; 
therefore, require additional x-rays to be taken.  So, great care needs to be 
given when these aprons are use, esp. with low lumbar curves).  I find it 
very sad that x-ray techs do not generally take these concerns into 
consideration.  When I was a teenager (wearing a Milwaukee Brace), I did not 
know about this issue of shielding reproductive organs (I am a male, and now 
 39 years old).  When my mother learned that I was not being shielded, she 
 raised hell!!
  
 As an aside, I have a copy of an article that my orthopedic surgeon wrote 
 for a professional publication back in 1979 that deals with this issue.  I 
 forwarded it to a number of people on this list a number of months ago.  If 
 you (or others) that were not on the list at that time, and are interested 
in 
 a copy of that article, please email me.

Hi all!  I was wondering if others that have had spinal fusion surgeries, and 
live on the East Coast of the United States or Canada found that they were 
unusually effected by Hurricane Floyd as he traveled up the East Coast this 
week.  I found that I was feeling achy and just plan "Yuk" from about midday 
on Wednesday until late into the evening on Thursday.  It was like I was a 
walking barometer, and it felt like I was able to feel every piece of metal 
in my back.  The Federal Government here in the Washington, DC area was given 
what is called Liberal Leave/Unscheduled Leave on Thursday, and I ended up 
staying home and slept until 3:30PM :-) !!  I would like to hear how others 
may have been effected before, during and after the storm.  Thanks!!

I am not sure if it is the Oxycontin, but I have lost my appetite pretty much,
hardly eating at all (I know, not good or particularly healthy in this
recovery period!) and have had some queasy feelings often.  Might be, in my
case, narcotics in general, as I also take Percocet.

Craig, Allison and all,

Got the approval to go to Cedars-Sinai Pain Center to see a Dr. Rothman for
pain management!  Hooray!  Will have to wait until Tuesday to make an
appointment due to Yom Kippur (yes Barbara, you were right!), but hey, we are
moving in the right direction.

My only hesitation, and please forgive me for sounding so much like a Negative
Nelly, is that this is the same outfit that provided pain management care
while I was hospitalized at Cedars, which was NOT a good experience for me. 
So will just have to hope that Dr. Rothman is a "good guy".  Dr. Pashman's
assistant seems to think so, as she said that she would trust her own family
to his care!

I was wondering if anyone on the List has had any experiences with
Cedars-Sinai Pain Center and/or any of their doctors; if so, please share with
me, thanks!

I have pretty much got my pain under reasonable control at this time.  I am
sure it could be better, but I am doing the best with what I have.  

This whole pain management thing is something that baffles and amazes me at
the same time.  That the care differs so much from patient to patient, from
doctor to doctor. This List has certainly proved that!

Thanks everyone for your input and support thru my battle with pain.  It does
seem all-consuming at times, especially when it is not controlled properly!

such wonderful things about Dr. La Grone. I just wish he was in LA so I could

I had my 75 degree curve reduced to 35 degrees.  My doctor, Dr.
Pashman/Cedars-Sinai Medical Center, Los Angeles, says that it is a great
correction.  But when I hear your numbers, I am wondering if mine is so great
after all.  How does one tell?

And you got 3 inches in height, how FABULOUS!

I can't begin to tell you how GOOD it is to read your message and hear that
you are so happy with your results!

Hello Jill,
Sometimes pain relievers do cause nausea.  I assume the other medication
you refered to is hydrocodone.  That as well can cause upset stomach.  It's
good to take them with food, or at least some milk and maybe a couple of
crackers if you get upset stomach.

Craig,

After developing three broken ribs during the summer of 1998 without any 
known trauma and a false positive on my testosterone level during initial lab 
work, I was referred to an endocrinologist for further testing.  This 
included a bone density (DEXA) test in early January of this year.  I posted 
detailed information about this back in July; however, I'll repost parts of 
it below as it should answer some of your questions.

I learned that bone density tests are scored based on age and standard 
deviations (SD) from the norm.  I am a 39 year old male; therefore, they used 
the young adult standards.  Normal is a bone mass density (BMD) greater than 
1 SD of the young adult mean; Low Bone Mass (OSTEOPENIA) is a BMD more than 1 
SD but less than 2.5 SD below the young adult mean; Osteoporosis is a BMD of 
2.5 SD or greater below the young adult mean; and Severe Osteoporosis is a 
BMD of 2.5 SD or greater below the young adult mean, in the presence of one 
or more fragility fractures.  My results showed moderate osteopenia as my 
forearm was -2.38 SD 
and my hip was -1.90 (although they usually scan the spine, they could not 
get a reading due to my spinal fusion instrumentation).  My endocrinologist 
said that mine was technically osteoporosis due to the very low reading on my 
forearm scan and the broken ribs.  I have been taking Fosamax, about 2000 mg 
of calcium, and vitamin D daily since January.  Two lab work follow-ups have 
found my testosterone and all other tested factors normal.  The current 
diagnosis is Idiopathic Osteoporosis. 

I would suggest that you discuss your concerns with your GP and Orthopaedic 
Surgeon, and possibly get a referral to an endocrinologist (I believe that 
men with osteopenia/osteoporosis issues are generally referred to an 
endocrinologist whereas women are treated by their gyn).  Hope this helps!!

Richard,

   I live just up the road from you in Columbia, MD, so we had about the
same weather last week.

   I did not really notice much effect from the huricane on my back (~3
months post op).  Most days I have varying degrees of discomfort, so it
might be hard to associate any pain I had specifically to the weather.  I
was aware that changes in pressure, etc. might cause me some discomfort, but
there was nothing dramatic to note.

   Now if I can just figure out how to get rid of all the branches and
leaves down in my yard!!
